## Telehealth Services

Teleios Functional Wellness, PLLC provides functional medicine, BHRT, and weight-management telehealth services within the state of Florida. All services are provided by licensed healthcare professionals. Services are not intended as a replacement for a primary care provider.

## Patient Responsibilities

You are responsible for providing accurate and complete health information, maintaining the confidentiality of your account credentials, ensuring a private environment for telehealth visits, and following the plan agreed to with your provider. Misrepresentation of health information may affect the quality of care provided.
